Online Parent/Guardian Survey – 2023
YOUR VOICE MATTERS!
Medfield Outreach and the Medfield Cares About Prevention (MCAP) Community Coalition are conducting an online parent/caregiver survey to better understand parental/guardian views as they relate to youth substance use in our community. This information will be used to inform programming to improve the health of our community’s young people. This survey is open to all Medfield parents and guardians. The survey should take less than eight minutes to complete. No personally identifying information will be collected during this survey, and your responses will remain completely anonymous. The data will only be reported on in aggregate form. We will not publish any individual responses to the survey.
